Registration and Waiver

A separate Registration and Waiver Form must be completed and signed by each person participating in the Memory Walk.


Please join us in helping BPUSA in its mission by asking family, friends and businesses to back you with pledges to support our work. Whether you walk one mile or a hundred feet, or even make a pledge is NOT important. What matters is that you have taken the time to join with family and friends - new and old - who are walking the same journey as you. To set up a personalized website to use for asking for donations from family and friends, go to this site: https://www.classy.org/campaign/walk-towards-hope-and- healing/c162565. Join your chapter’s team, create your own team or set up an individual page.

A sponsorship page is included in this packet, if you prefer to ask family and friends for donations, instead of using the website.

We would like to have all chapters participating in the Walk make a banner with the names of the children in your chapter to be carried on the “Walk Towards Hope and Healing” at the Gathering. If you are not a member of a chapter, individuals can also make and carry a banner or sign with your child’s name and/or picture.

For a donation of $50 a sign with your child’s name and photo to be placed along the walk route, the sign will be yours to take home at the end of the walk. Photos submitted for the slide show will be used on the t-shirts and signs.

Deadline to submit registration form in order to receive a t-shirt with picture or have a sign made is July 1, 2018.

LIABILITY WAIVER MUST BE SIGNED BEFORE PARTICIPATING IN MEMORY WALK 

WAIVER AND RELEASE: I recognize that participation in the BPUSA Memory Walk may involve certain hazards. I understand that I should not participate unless medically able. I assume all risks associated with involvement in this activity, including but not limited to falls, contact with participants, the effects of weather, including high heat and humidity, the conditions of the track and/or road, traffic on the course, and all risks being known and appreciated by me. Having read this waiver or release, knowing these facts and in consideration of my acceptance into this Memory Walk, I, for myself and anyone entitled to act on my behalf, waive and release The Bereaved Parents of the USA, and all sponsors and hosts, and their representatives and successors from all claims or liabilities of any kind arising from involvement in this activity.
